To have a helthy enveromment and to help ill ppl to rehabitation thier skills mjsdn is dedicated to the patient care philosophy of “continuity of care." We believe that the best care can be given to a patient by staff members that know the patient well, so we always try to send the same clinical staff to the home of each client we care for. Day to day changes are noted sooner when the same staff members give patient care. It helps patients and family members feel more relaxed when they can rely on the same clinical staff to provide care to their loved one. Continuity Care Home Nurses provides patients care at home (private duty and/or intermittent visits) and  in the hospital (and other settings as requested depending upon the patient’s clinical status and orders from the M.D.)
